INSTRUCTIONS: Identify the environment and activity. Write down all of the steps that are needed to perform the activity by a nondisabled peer in the left column of the form. Then have the student perform the activity. Mark (+) if the student can perform the skill independently and (-) if the student can't perform the skills independently in the column marked (+/-). Write notes in the comments column about cues that were given so that the student could perform the step. Also note any supports that could be used or any skills that might be taught to help the student perform the step.
