PHASE 9 SIMULATION - NICK

In the course of Phase 8 - Determine Adaptive Supports And Needed Skills for Travel, the team developed ways to help Nick be a more independent shopper. They took each discrepant step, came up with a specific solution and then proceeded on to Phase 9.

In Phase 9 - Develop a Travel Plan, the team described Nick's support modifications and strategies. The travel plan sketched out what, when, where and how the activity would unfold. It also identified how often the team intended to measure Nick's progress. Once the travel plan was outlined, the team proceeded on to Phase 10 - Implement the Travel Plan and Assess Progress.

TRAVEL PLAN

Student: Nick School Year: 2000-01 Area of Need: Community

Annual Goal: Nick will, to the best of his ability, actively participate in community activities with his family.

Activity Setting: Supermarket

Activity Environments: Prep at home, ride in car with Mom, get adaptations ready next to shopping baskets, locate the taco shells in aisle 8, and pay at the express checkout.

Activity Sequence:

Arrive at supermarket

Go to shopping baskets

Get adaptations and shopping basket ready

Look for aisle 8

Find Mexican food section

Get Taco Bell brand taco shells

Go to express lane check out

Pay for purchase

Exit store

Return home

Activity Objectives:

1. Independently walk throughout store carrying a shopping basket,

*2. Keep pace with Mom as she walks through store environment,

*3. Learn to recognize the location of aisle 8,

*4. Learn to recognize the location of the Mexican food section,

*5. Learn to match a picture symbol with its associated object,

*6. Learn to find the express lane checkout,

7. Independently get out wallet and place it in basket,

8. Independently get in line and put basket on conveyor belt,

9. Independently complete purchase and pick up plastic bag,

10. Independently recognize the location of the store exit.

Activity Timeline:

8:00am: prep for shopping

8:10am: leave for store

8:20am: enter supermarket

8:25am: organize adaptations and get shopping basket

8:30am: find aisle 8 and Mexican food section

8:35am: locate Taco Bell taco shells

8:40am: locate express lane and get in line

8:45am: complete transaction with cashier

8:50am: leave supermarket

9:00am: return home
