Phase 9 Simulation

PHASE 9 SIMULATION - KRYSTAL

In the course of the BETWEEN portion of Phase 8 - Determine Supports to Address Discrepant Steps, the team decided Krystal should wheel herself through every doorway she encountered. In the course of the WITHIN portion of Phase 8, the team listed the supports Krystal would need to actively participate in two separate activity sequences. They then proceeded on to Phase 9.

In Phase 9 - Develop a Travel Plan, the team put together an overview of Krystal's vocational placement at the hospital and then proceeded on to Phase 10 - Implement the Travel Plan and Assess Progress.

TRAVEL PLAN
Student: Krystal School Year: 2002-03
Activity Setting: Hospital
Activities and Environments: Prep in classroom, leave school, ride in taxi, enter hospital building, put away coat in volunteer office, travel to do job task in Central Supply, travel to buy yogurt snack in cafeteria, travel to retrieve coat in volunteer office, leave hospital, ride in taxi, and re-enter school building.

Activity Timeline:
12:00 prep for work at school
12:10 leave school classroom
12:20 catch cab
12:30 arrive at hospital entrance
12:45 check-in at volunteer office
1:00 enter the Central Supply area
1:10 begin work at job site

2:30 leave the cafeteria
2:40 catch cab
2:55 return to school

Activity Objectives:
1. Bear weight during transfers from wheelchair into taxi cabs.
2. Wheel through doorways at school and hospital.
3. Wheel onto and off of elevator at hospital.
4. Wheel to and from work table and IV boxes in Central Supply
5. Wheel to food area, cashier, table and trash in the cafeteria.
