PHASE 9 - DEVELOP A TRAVEL PLAN
Develop a Travel Plan
Once the team has decided how discrepant steps will be addressed, team members will need to put the entire activity sequence into a travel plan. The plan should outline any specific objectives, the measures for determining progress, and how frequently the data will be collected and reviewed. In addition, the plan should include steps to complete the route or activity, materials to be used, where supports including adaptations are necessary and who will be responsible for providing assistance, supervision, and/or instruction along the way. It is important for the team members to provide the student or consumer enough opportunities to actually learn any skills or use any necessary supports.

STEPS TO COMPLETE
1.
Write the objective(s) including the appropriate progress measures and data collection schedule.
2.
Outline all the steps of the activity or route.
3.
List all the necessary materials, supports and adaptations required and where in the step-sequence they will be needed.
4.
Determine who will be the person(s) primarily responsible for supervision, assistance, and instruction.
5.
Decide on a schedule for participation in the activity or route.

OUTCOMES
1.
The team will develop agreed-upon objective(s) with the means to measure improvement or assess benefit.
2.
A plan is developed that includes a list of steps in the activity or route, including all skills, necessary materials, supports and adaptations required and where in the step sequence they will be needed.

3.
A schedule for participation in the activity or route is developed.

TIME ALLOCATED
Steps 1-5 may take at least 30 minutes for each objective.
