PHASE 10 - IMPLEMENT THE TRAVEL PLAN
AND ASSESS PROGRESS
Implement the Travel Plan and Assess Progress
The team needs to determine how well the Travel Plan is working. Team members will need to judge both the progress of the person who is deafblind to actively participate (i.e., learn skills, use adaptations, understand his or her role and/or activity expectations), as well as the appropriateness of any supports provided. To gauge the overall success of the Travel Plan, team members may want to use a combination of anecdotal material (e.g., observations recorded by various team members) and objective data collection at specific steps along the route or during the activity (e.g., length of time to complete the activity). After collecting data over a period of time, the team will need to evaluate how the person is doing and what, if any, changes may be needed.

STEPS TO COMPLETE
1.
Develop or identify any necessary data collection forms.

See example Data Collection Form A (blank) and Data Collection Form B (blank).
2.
Determine the data collection schedule and collect information over a predetermined length of time. Initially, team members may want to collect data every time the person completes the route or takes part in the activity. Later the team may want to collect data only once per week.
3.
Review the data and the Travel Plan to decide if the person is making progress and make changes as needed.
4.
Once the student or consumer is successful in the activity/route option, the team may want to use the model to select another environment, activity, or route.

OUTCOMES
1.
Data collection forms are used.
2.
A variety of data is collected and used to measure progress toward stated objectives and to determine necessary changes in activity or route supports and skills.
3.
The use of the model and data collection procedures results in more active participation by the individual who is deafblind.

TIME ALLOCATED
Based on the data collection schedule and the activity or route, the time to complete Steps 1-4 will vary.
