Annual Goal: Krystal will independently move her wheelchair short distances during and between activities occurring throughout her daily schedule.

Criterion: The goal will be met when Krystal can wheel independently to or during activities in her daily schedule for 8 out of 10 opportunities. (Data will be collected for all steps of the travel sequence(s) using a data collection form. Progress will be calculated at the end of every month.)

Materials: object communication symbols on her fanny pack, object cues at activity locations, one-wheel drive wheelchair

Person(s) Responsible: Mr. Move, PT; Ms. Learned, classroom teacher; Ms. Assistor, paraeducator

Time Allotted: Approximately 15 minutes to art class destination, but time allotment will vary for other activities.

Steps: (the sequence of steps for the first activity of the day)

  1. 1. Krystal obtains object symbol for the selected activity/location from her calendar box and places it on her fanny pack with physical assistance.
  2. 2. Krystal’s wheelchair is pushed through the doors to the hallway. Krystal assists with pushing open the classroom door.
  3. 3. Krystal is able to move her wheelchair using one hand and will be expected to propel her wheelchair down the hall. Some prompting may be required to start and to indicate direction.
  4. 4. With hand under hand assistance, Krystal will use one hand to trail the wall to her destination (Fade hand under hand assistance as proficiency increases.)
  5. 5. Krystal will locate the appropriate classroom door by matching the object symbol from her fanny pack with the one at the door. Krystal assists with opening the door and her wheelchair is pushed into the classroom.
  6. 6. Krystal puts her object symbol in her fanny pack.
  7. 7. Steps 1-6 are repeated for the other daily travel destinations/activities.

Reinforcement: Give positive reinforcement for each step of the sequence by patting her on the back. Fade positive reinforcement as proficiency increases. Then, reinforce her using verbal praise and patting her on the back when she reaches her final destination.

Generalization: Once Krystal reaches her goal, data will be collected two times a month to see if her skills are being maintained. If they are, determine if there are other activities
